This web-form serves as an on-line account registration interface. The very first drop-down box links to Batch Add Settings in YPMan, which defines courses and their home-directories. Note that SSN is not a requirement for YPMan, but rather serves a a unique identifier - it may as well be an employee number, or the like.
Successful registration only takes place, if all fields have been entered correctly.
